Pickleball Waiver Form

The contact person of each team should download this waiver form for each player to sign and return to the tournament organizer prior to the first match. No player can participate in the tournament without having this waiver signed and turned in.

Pickleball Tournament Guidelines
Each division will be capped at 20 teams.

The tournament will be a 3gg (3 game guarantee) double elimination.
  
MD will provide the balls, but players must have their own paddle.

No individual will be allowed to play in both social and competitive level of play
